Do you have a chronic condition like arthritis, diabetes, overweight, high blood pressure, anxiety, or chronic pain?
Living Well with Chronic Conditions is a self-management workshop that presents a variety of helpful tools that improve the many common symptoms of chronic conditions like pain, frustration, anxiety, sleeplessness, fatigue, etc. Trained volunteer leaders teach participants to develop individualized action plans and they ,along with classmates, provide mutual support for implementing plans and making positive lifestyle changes.
Tools includes pain management, improving sleep, nutrition, exercise, stress management, making decisions, and many more in six, weekly sessions for 2 ½ hours each.
It is free outside of a $10 optional book. If you would like to attend, please fill the attached form and return as soon as possible to the Bayfield County Health Department at firstname.lastname@example.org. The class will be open to employees and the public (up to 12 persons max). Please also see attached brochure and feel free to share it with family and friends!
